What is CMD

Many complaints. One possible cause.

Headaches. Tinnitus. A neck that never settles. Exhaustion that no night's sleep takes away. And everywhere the same answer: “There's nothing there.” If that is your story, please read on. There is an explanation that is almost never checked along the way.

Diagram: a standing person with ten levels of the body labelled around them,
from the feet up to the brain. Typical complaints are listed for each level,
such as pain in the soles of the feet, knee pain, pelvic obliquity, back pain,
shoulder tension, neck pain, jaw joint pain, tinnitus and headaches. Lines
connect every level with the jaw system. The labels in the diagram are in
                    German.

Do these sentences sound familiar?

  • “It's psychological.” Or: “Stress is to blame.”
  • “You'll have to live with it.” Without anyone having looked for the cause.
  • Sent from doctor to doctor, and nobody sees the whole picture
  • The feeling that nobody really listens to you
  • Treatments whose promises were not fulfilled
  • And the quiet fear that something serious is behind it after all

If you have nodded more than once: your complaints are real. The fact that nobody has found anything so far does not mean that nothing is there. It can mean that people were looking in the wrong place.

The explanation nobody checked

Why CMD goes undetected for so long

Each specialty examines its own organ and finds nothing there. The headache belongs to neurology, the ear to ENT, the neck to orthopaedics, and in the end the exhaustion to the psyche. But nobody looks at the system that is connected to all four.

Through muscles and nerves, the jaw system is linked to the head, ear, neck and spine. If something there is out of rhythm, it rarely shows up in a single place. It shows up in several. Never in the one where the cause lies.

That is exactly what makes CMD so hard to pin down. And that is exactly why what you have experienced is not a failure on the part of your doctors. Each of them looked properly – only within their own specialty.

The term, decoded

What CMD is

Cranio

Skull

the skull with the jaw joints

Mandibular

Lower jaw

the position and movement of the lower jaw

Dysfunction

Misregulation

the interplay loses its rhythm

Put together: a reflex, nerve-driven misregulation in the interplay of the jaw joints, the jaw musculature and head posture. Small at the cause, large in its effect. In English-speaking countries the condition is often called TMD (temporomandibular disorder).

CMD Systems Medicine: why we do not talk about repair

The word misregulation is chosen with care. It is not about a part that is broken and has to be replaced, but about a control system that is set wrongly. Control systems can be readjusted. That is the whole difference. And it is the reason why we do not talk about repair, but about regulation.

The patterns of complaints

Do you recognise yourself?

What looks like several different illnesses is often one problem with many faces. Look for what fits you.

Tinnitus, ear pressure & dizziness

Does this sound familiar? A whistling or rushing sound that is loudest at night. A feeling of pressure, as if the ear were blocked. Dizziness or unsteadiness. And at the ENT specialist “everything was fine”.

Why nothing has helped you so far: when the ear is examined on its own, the cause remains invisible. Because it does not lie in the ear, but right next to it.

The connection: the jaw joint lies immediately next to the ear. Tense jaw muscles and an unfavourable joint position can affect the structures of hearing and balance. In detail →

Jaw clicking & teeth grinding

Does this sound familiar? It clicks or grates when you chew and yawn. In the morning your jaw is stiff and your teeth are sensitive. Your partner hears you grinding at night.

This is the clearest warning sign: grinding and clenching are the most direct signs of an overloaded jaw system – and often the driving force behind head, ear and neck complaints.

The connection: here the cause is where it all happens. Restricted or crooked mouth opening shows that the joint and the muscles are not working in balance. In detail →

Neck, back & posture complaints

Does this sound familiar? Stubborn neck and shoulder tension. Back or hip pain that keeps coming back. Massage and physiotherapy feel good, but the complaints return.

A possible reason: physiotherapy and manual therapy work, and they are the first choice. We do not change that – we work together with them. If the complaints still keep returning, it is worth asking whether something further up is permanently pulling against them.

The connection: the cervical spine and the jaw system are controlled by shared reflexes; treating the neck as well is professional standard in CMD. In detail →

Emotional strain & inner tension

Does this sound familiar? Low mood, irritability, sleep problems, the feeling of being at the end of your strength. On top of that, the worry that a serious illness is behind it all after all. And a burden that has long since reached every area of life: work, family, friendships, your own plans.

This is almost always read the wrong way round. As a rule it is not the psyche that produces the pain. It is the other way round: a physical misregulation produces an emotional burden over the years, because constant pain wears you down and because nobody finds anything.

The connection: serious causes must always be investigated by a specialist. Once they have been ruled out, a functional misregulation of the jaw system is a common, benign and readily treatable explanation. This certainty alone takes away the fear for many. We take both seriously and also provide psychological support along the way.

Exhaustion & lack of energy

Does this sound familiar? You are worn out, drained. You wake up unrested in the morning and drag yourself through the day as if you had never really slept. Your reserves are permanently empty.

Why nothing has helped you so far: exhaustion is quickly dismissed as “stress”. If the physical source is not looked for, it remains untreated.

The connection: grinding at night and permanently overloaded muscles do not let the body come to rest at night. Sleep does not restore you, and the constant tension drains your strength.

Several complaints, no findings anywhere

Does this sound familiar? Several complaints at the same time that no single specialty can explain. A long series of examinations, and at the end always: “There's nothing there.”

This is exactly where a look at the jaw system is worthwhile. Because it is connected to so many areas, CMD rarely shows itself in a single place. The number of your complaints is not a sign that nothing was found. It is itself the clue.

What this means for you

The good news

It can be found

Function can be measured, not just guessed at: bite position, muscle activity, movement. What has been measured is then there in black and white.

It can be treated

In the vast majority of cases CMD is readily treatable, with removable appliances, without anything being altered on healthy tooth structure.

It is benign

A misregulation is not a progressive disease. For many, this alone is the greatest relief after years of uncertainty.

The most important step is the first one: recognising it at all.

So that you hear it from us

What is established and what is not

We could promise you more than we can prove. We do not, and we will also tell you why: you have heard enough promises.

Well supported professionally

  • the connection to the head, face and neck; treating the neck as well is professional standard
  • the association with migraine and with ear complaints
  • the connection between sleep quality and CMD
  • that first-line treatment is conservative and non-invasive

Not proven – our observation

  • the link to the spine, pelvis and foot posture: the data are inconsistent
  • a causal connection to tinnitus and dizziness; what is proven is an association, not a cause
  • whether and how much your complaints will change. That differs from person to person and cannot be said in advance

And something obvious that we write down anyway: the same complaints also have other causes. If they persist, they need to be investigated by a doctor or dentist. Sudden hearing loss, acute dizziness or new neurological signs always come first, and immediately.
